Black spot on belly near vent. Impaction?

I have a male leopard gecko about 2 and a half years old. He has not eaten for about 3 weeks now and I recently noticed a black spot on his lower belly. The area is starting to look very swollen. The last few days, I soaked him in warm water and massaged his belly trying to see if maybe it was impacted poop I could push down, and the spot seems to be lower than it was initially but it has not gone away yet. I have him housed in a 10 gallon glass tank and he has a warm and moist hide with plenty of water. I recently switched him from a 75 watt black night light to an under tank heater. He always pooped regularly before, but now this problem has arised and I don't know if it has anything to do with switching to the heater but I thought I would put that out there. His substrate is Repti carpet and I have been feeding him mealworms dusted with calcium and multivitamins. He has always been a great eater and never had problems with digesting or shedding. I have a few pictures, but it was kind of hard to take a picture with one hand while holding him, but you can see where the spot is and you can see some of the swelling. I took these pictures after I took him out of the water. His tail just started getting thinner as well since he has not eaten, but it has never been that thin. I just want to know whether this is impaction or some other issue and what I can do to fix it and prevent it from happening.

That doesn't look like an impaction to me, and it doesn't sound like there is anything for him to get impacted with.

Honestly to me it looks like internal bleeding, mostly because of the red that looks like it has spread across the area.

Are you running your UTH on a thermostat? I suppose it could be a burn if you're not.

Either way, I highly encourage a vet visit. That doesn't look good and looks pretty painful.

I'm honestly not sure what the problem is but I know it isn't a burn, so I guess it could be impaction or the bleeding.. I'll be taking him to the vet asap and I will update when I find out more.
