Failure to thrive or sick?

We have a young leopard that we got from a pet store. She hasn't grown very much in the 4 months we have had her. I've tried just about everything. We took her to a herp vet 2-3 weeks ago. He treated her for parasites. Gave one dose of something there and sent us home with an antibiotic. She did great for 1-2 weeks and her tail was getting fatter. Now, her tail is the skinniest it's ever been. Thinner than a pencil. The vet said she weighed 10 grams and was 20% under weight for her age. Her poop often has the whole bug still in it. I'm guessing she isn't breaking food down and absorbing the nutrients? Any advice would be appreciated. She's my 10 year olds pet and my daughter is very upset and worried.
We are feeding Dubai roaches and Phoenix worms with crickets here and there. Ty!!

So, we've had her almost 3 months now. I was off on my time in the first post. She was very young when I got her. She still looks like this. Tail and all.

Sometimes the first dose of wormer only kills live worms but there are still eggs and it doesn't kill them. They hatch in a few weeks then they got worms again. It takes two doses at least to get rid of worms and then a follow up once every few months.

I have an email in to her vet and will call tomorrow.
I've tried to get a fecal sample for them to test, but she poops during the night and the heat lamp has dried out too much by morning to test it. I may set an alarm and wake in the middle of the night to collect some. Ty!

Shrapnailed, we were giving wax worms, but I read the Phoenix worms are better and higher in calcium, etc.
I'll grab some wax worms tomorrow and give her variety.

Mine live off of Meal worms and Crickets with the occasional treat of wax worms. I have some new photos that I have to upload to here so ill add you so I can show you how mine look

Mine looks anorexic compared to pictures I see of 3/4 month old leopards. She's not fully skeletal like some pictures I see of sick tail geckos, but she's super skinny.

If she were gaining and thriving like normal, I would have stuck with the crickets, etc. I'm just trying everything I can to fatten her up.
I found some stuff by Flukers to help when they are underweight like this. It's a boost. Would any of you recommend something like that or know of one that is beneficial?
