Leo wont eat

Iv had me leo for about a month now i bought it as a hatchling from pet. co and i think i may have boughten a sick one. Everything in my tank is right... tile floor, hot side at 93 degrees, cool side at 75, Warm hide, cool hide, moist hide, fresh water everyday, I offer it crickets and meal worms every night before bed dusted with repashy calcium plus and my leo will NOT eat... since iv had it iv only seen it eat 1 cricket and a few worms... the gecko has lost a ton of weight and i fear he only has short time left. Iv done everything right and i cannot get it to eat... any suggestions? please help!

This may help, it's a slurry, put it in gecko's nose and she'll lick it up, don't clog nostril...
In a blender, combine:
1 small can Hill's A/D pet food (available at most vets)

1/4-1/3 cup Ensure (not chocolate)

1 jar baby food squash -

1/4-1/3 cup Pedialite

2 crushed tablets or contents of 2 capsules Milk Thistle (herbal supplement, liver purifier, found at most drug stores.)

1/2 tsp. each calcium powder and herp vitamin powder -

1-2 extra large handfuls of mealworms


Also when she ate did she struggle?, like did the mealworm keep falling of her mouth, if so she may have rubber jaw, to cure this mix calcium and water into a paste and also put it in her nose so that she can lick it, this should fix it
