- How long have you owned your leo: 1 month
- Where was he/she obtained: Local exotics store

A) Health/History
- How often do you handle your leo: Every day or every other day
- Is your leo acting any different today? If so how does he/she normally act which differs from now: She is a little more skittish than normal
- Has he/she had any problems in the past, if so please describe: NA

B) Fecals
- Describe (look any different than normal): Well formed. Brown with urate
- When was the last time he/she went: Few days ago
C) Problem:
She has vomited three times in the last two weeks. The first two I am guessing was because she over ate but she also vomited a few waxworms


Housing:
A) Enclosure
- Size 20 gal
- Type glass tank
- Type of substrate: repti carpet
- Hides: 4 - 1 moist, 1 dry and hot, 2 dry and cool
B) Heating
- Heat source: Heat lamp and a small UTH
- Cage temps : 85-90 cool side: 70
- Method of regulating heat source: Daily cycle
- What are you using to measure your temps: Therm probe
- Do you have any lights : overhead day lamp
C) Cage mates
- How many: 1 male
- Describe health: Very healthy and active

Describe Diet:
A) Typical diet
- What you're feeding : 5-10 mealies every few days
- How are you feeding: tong fed
B) Supplements (describe how often)
- What vitamin/minerals are you using: Calcium with D3 (ZooMed I believe)
- What are you gut loading food with: Fluckners gut load diet/ oatmeal w/ carrots, kale and apple
